DEPRESSION

Unfortunately there is no brain scan or blood test that can be used to diagnose when a person has a depressive illness. The diagnosis can only be made from the symptoms. Generally speaking a diagnosis of depression will be made if a person has a persistently low mood that significantly influences their everyday life and has been present for two weeks or more, and there are also three or four or more other symptoms of depression.
Doctors make a diagnosis of depression after assessing the severity of the low mood, other associated symptoms and the duration of the problem.

Causes:
We do not fully understand the causes of depression.

  • Genes or early life experiences may make some people vulnerable.
  • Stressful life events, such as losing a job or a relationship ending, may trigger an episode of depression.
  • Depression can be triggered by some drugs.
  • It is often impossible to identify a 'cause' in many people and this can be distressing for people who want to understand the reasons why they are ill. However depression, like any illness, can strike for no apparent reason.

It is clear that there are definite changes in the way the brain works when a person is depressed:

  • Modern brain scans that can look at how 'hard' the brain is working have shown that some areas of the brain (such as at the front) are not working as well as normal.
  • Depressed patients have higher than normal levels of stress hormones.
  • Various chemical systems in the brain may not be working correctly including one known as the serotonin or 5-HT system.
Symptoms of depression:

Stress can lead to you to feeling 'down' and 'miserable'. What is different about a depressive illness is that these feelings last for weeks or months, rather than days. In addition to feeling low most or all of the time, many other symptoms can occur in depressive illness (though not everybody has every one).

  • Being unable to gain pleasure from activities that normally would be pleasurable.
  • Losing interest in normal activities, hobbies and everyday life.
Feeling tired all of the time and having no energy.

  • Difficulty sleeping or waking early in the morning (though some feel that they can't get out of bed and 'face the world').
  • Having a poor appetite, no interest in food and losing weight (though some people overeat and put on weight - 'comfort eating').
  • Losing interest in sex.
  • Finding it difficult to concentrate and think straight.
  • Feeling restless, tense and anxious.
  • Being irritable.
  • Losing self-confidence.
  • Avoiding other people.
  • Finding it harder than usual to make decisions.
  • Feeling useless and inadequate - 'a waste of space'.
  • Feeling guilty about who you are and what you have done.
  • Feeling hopeless - that nothing will make things better.
.
Treatments:

Sometimes when we are going through a 'bad patch' in our life, it is enough to talk through our problems with a friend or relative. However, this may not be enough and we may need to seek professional help. The important thing to remember about depression is that it is treatable. There are many different types of treatment. These include medication and talking therapies (psychotherapy).
Medicine:

    • Antidepressants helps to correct the 'low' mood and other symptoms experienced during depression; they are NOT 'happy pills'.
    • Antidepressants do not change your personality.
    • Antidepressants are NOT addictive.

In the last few years there has been an explosion of new antidepressant medications. The main advantage of these new drugs is that they have fewer side effects than older drugs and so are more pleasant to take.

Your doctor will choose which medication to prescribe for you based on the side effects of the drugs and your particular symptoms. We do not fully understand how antidepressants work. However, they appear to act on chemicals in the brain to correct the abnormalities which cause the illness.

When taking medication it is important to remember:

    • current World Health Organisation guidelines recommend that patients continue to take their medication for six months after having recovered. This is to prevent a recurrence of the illness when the medication is stopped.
    • antidepressants are effective for both treating episodes of depression and also for preventing further episodes of illness. Some patients who have had severe and/or many episodes of illness are therefore recommended to take medication for a long time.
    • stopping to take medication once you feel well is a common cause of a return of the symptoms of depression. You should therefore only stop after discussion with your doctor.
    • generally coming off antidepressants is not a problem, though usually you should gradually reduce the dose of the medication over a few weeks rather than stopping abruptly.

Classes of antidepressant medicines

  • Lithium
  • Monoamine oxidase inhibitors
  • Selective serotonin re-uptake inhibitors (SSRIs)
  • Tricyclic antidepressants (TCAs)
  • Other antidepressants
    • Noradrenaline re-uptake inhibitors (NARIs)
    • Noradrenergic and specific serotonergic antidepressants (NaSSAs)
    • Reversible inhibitors of monoamine oxidase type A (RIMAs)
Serotonin noradrenaline re-uptake inhibitors

All depressions indicate some vital organ is loosing vitality and in distress though the symptoms may not appear outwardly....doctor dignose the disease only when it is deteriorated or dysfunctional and perceptible... all 90% emotional imbalances are results of physical imbalances...

In some countries doctors are careless to diagnose the actual cause of depression, they just prescribe anxiolytic to the patient that is a wrong approch.

Late diagnosis of disease is not particular with depression. It is common practice, we give attention to the disease when symptoms appear.

Late diagnosis of disease is not particular with depression....depression itself is symptom ...and manifested after long slow buildup of the problem inside body or mind....it doesnt appear suddenlu out of nowhere..so in that sense it is late...

The biogenic amine theory describe well the basis of the disease that is the depletion of norepinephrine and serotonine.

what is the cause?
Despite so many causes of depression, if we focus on the biological basis for depressive disorder we find the decrease level of norepinephrine and serotonine. Although it is not the only cause. if we look at mechanism of action of anti depressant drug (SRRI) they ultimately increase serotonine level.

Despite biological causes there may be some social causes as well like stressful life events, such as losing a job or a relationship ending, or some failure, may develop the state of depression.

same social cause, failure, relationship ending creates in one man depression while to other nothing happens.that means every cause has an effect but results are produced depending on person's strength and many other factors.so traetment should not be based on cause alone without taking into consideration the disposition, inclination and tendencies into account.i feel the treatment should be reason based then cause based.
and further the reasons should be found out why one person has a disposition towards depression while others do not have.social causes, relationship ending, failure etc causes are all outword circumstances which give apportunity to person's being to manifest what is hidden in the form of disease or strength.

Some times we used the word of depression for its symptoms like depressed mood, feeling of worthlessness or excessive or inappropriate guilt, diminished ability to think or concentrate or indecisiveness, loss of interest or pleasure, etc.these are symptoms of depression.

"The treatment should be reason based then cause based" It is idealistic thinking, is it possible in practical life?
Reasons>> causes>> symptoms>> disease state>> treatment, This is the way a disease is appeared. physician correct the causes of disease usually by medicines. Suppose a patient of depression, the reasons
of depression is the job lost, can a physician re appoint him on the job? No. because it is beyond to his ability, so he do the job what he can-

Not able to face circumstances...inside fear...inside resistance...under confidence...and we may go beyond ... these are all the symptoms of social phobia. What so ever the reason of the disease, inward or outward patient must taken for the treatment.

The most important thing anyone can do for a depressed patient is to help him get to a doctor for a diagnostic evaluation and treatment. First, try to talk to him about depression, help him understand that depression is a common illness among men and is nothing to be ashamed about. Then encourage him to see a doctor to determine the cause of his symptoms and obtain appropriate treatment.

Once he is in treatment, you may continue to help by encouraging him to stay with treatment until symptoms begin to lift (several weeks) or to seek different treatment if no improvement occurs. Encourage him to be honest with the doctor about his use of alcohol and prescription or recreational drugs, and to follow the doctor's orders about the use of these substances while on antidepressant medication.

The second most important thing is to offer emotional support to the depressed person. This involves understanding, patience, affection, and encouragement. Engage him in conversation and listen carefully. Do not disparage the feelings he may express, but point out realities and offer hope. Invite him for walks, outings, to the movies, and other activities. Be gently insistent if your invitation is refused. Encourage participation in some activities that once gave pleasure, such as hobbies, sports, religious or cultural activities, but do not push him to undertake too much too soon. The depressed person needs diversion and company, but too many demands can increase feelings of failure.

Do not accuse the depressed person of laziness or of faking illness, or expect him 'to snap out of it.' Eventually, with treatment, most people do get better. Keep that in mind, and keep reassuring him that, with time and help, he will feel better. A sincere friend can encourage him to face the problems of life, and improve his behavior to the run the social life.

Not able to face circumstances...inside fear...inside resistance...under confidence...and we may go beyond ... these are all the symptoms of social phobia......to give names to the problems helps to deal with the treatments by doctors to diagnose ...but real problem is reason of phobia...doesnt matter what phobia...

If someone is not able to face circumstances, because of inside fear, inside resistance, or he is under confidence; then how would you solve this problem?

yes that is the real question....so first to understand that diseases are not coming from outside but inside.....outside gives only circumstances to manifest what is inside....
so to go in inner being our mind should go inward...with no thought and emotion...just be there...so that ur thought/emotion cannot condition what is inside...sooner or later u can percieve the problem or the problematic points...but i know everybody cant do this...that is why in tibetan medicine i heard...the doctors have occult powers to see ineer being...it is their essential qualification...only bookish knowledge is not enough.
but if u go deep inside then problem arises ...bcoz ur past is the reason...any past deeds and emotions...and i m talikng not only this life past but previous lives...i know muslim dont believe on past lives..so what ample proof around the globe are available for this including edgar cayce writings...there are astrological reasons too...but that too due to past...
all seeds od diseases are hidden in unconcious...u can say in technical term in genes...and at the right time as per astrological influence they manifest...
superficial diseases can be cured easily without going deep on the issues like accident, fracture etc...but chronic, mental etc disease require deeper understanding...
so in past lives if somebody has misused his power and people were afraid of him...hence this life as a resultant action he will be with fear bcoz he was the reason of fear for others...and so on...
so all doctors have limitations of solving the disease...they can suprress the diseased energy..but it will go somewhere else...in body or unconcious but this is not the solution...
and if person's deeds and past life are the cause then how doctor can solve the problem...he can only diminish the intensity or change the form of disease...all disease cure is persons responsibilities...he will have to learn certain things...then religion comes into picture...
health is not absence of disease but physical, mental and soul level simultaneously one is full of enrgy, tranqulity and joy....his state of mind will be inward not outward....
in hindu health is called swastha(swa--means khud or self and stha means established)...that means one who is established in his inner being..one who is centered...
